The injury
The navicular forms the keystone of the medial arch. It may be fractured by a direct blow or a twisting injury, and stress fractures occur in runners and jumping athletes, often presenting as poorly localised midfoot pain with normal initial radiographs.
Diagnosis and treatment
CT or MRI confirms the diagnosis. Undisplaced and stress fractures are generally treated with a period of strict non-weight-bearing in a cast. Displaced fractures and non-unions are fixed with screws, with bone graft where required.
